Frank E. Bird, Jr., who was then the Director of Engineering Services for the
Insurance Company of <st1:place w:st="on">North America</st1:place>. He was
interested in the accident ratio of 1 major injury to 29 minor injuries to 300
no-injury accidents first discussed in the 1931 book, Industrial Accident
Prevention by. H. W. Heinrich. <o:p></o:p></span><br />
<br />
<span lang="EN-US">Since Mr. Heinrich estimated this relationship and stated
further that the ratio related to the occurrence of a unit group of 330
accidents of the same kind and involving the same person, Mr. Bird wanted to
determine what the actual reporting relationship of accidents was by the entire
average population of workers. H.W. Heinrich’s classic safety pyramid is now
considered the foremost illustration of types of employee injuries.<a href="http://emeetingplace.com/safetyblog/wp-content/uploads/2008/07/clip-image0014.jpg"></a><o:p></o:p></span><br />

<span lang="EN-US">There Bird analyzed 1,753,498 accidents reported by 297
cooperating companies. These companies represented 21 different industrial
groups, employing 1,750,000 employees who worked over 3 billion hours during
the exposure period analyzed. The study revealed the following ratios in the
accidents reported: <o:p></o:p></span><br />

<span lang="EN-US">For every reported major injury (resulting in fatality,
disability, lost time or medical treatment), there were 9.8 reported minor
injuries (requiring only first aid). For the 95 companies that further analyzed
major injuries in their reporting, the ratio was one lost time injury per 15
medical treatment injuries.<o:p></o:p></span><br />

investigated all property damage accidents and eighty-four percent stated that
they investigated major property damage accidents. The final analysis indicated
that 30.2 property damage accidents were reported for each major injury.<o:p></o:p></span><br />

<span lang="EN-US">Part of the study involved 4,000 hours of confidential
interviews by trained supervisors on the occurrence of incidents that under
slightly different circumstances could have resulted in injury or property
damage. Analysis of these interviews indicated a ratio of approximately 600
incidents for every reported major injury.<o:p></o:p></span><br />

incidents discussed with the interviewers and not the total number of accidents

<span lang="EN-US">Bird continues, as we consider the ratio, we observe that
30 property damage accidents were reported for each serious or disabling
injury. Property damage incidents cost billions of dollars annually and yet
they are frequently misnamed and referred to as "near-accidents”.
Ironically, this line of thinking recognizes the fact that each property damage
situation could probably have resulted in personal injury. This term is a
holdover from earlier training and misconceptions that led supervisors to
relate the term "accident" only to injury.<o:p></o:p></span><br />

<span lang="EN-US">The 1-10-30-600 relationships in the ratio indicate clearly
how foolish it is to direct our major effort only at the relatively few events
resulting in serious or disabling injury when there are so many significant
opportunities that provide a much larger basis for more effective control of
total accident losses.<o:p></o:p></span><br />

was of a certain large group of organizations at a given point in time. It does
not necessarily follow that the ratio will be identical for any particular
occupational group or organization. That is not its intent. The significant
point is that major injuries are rare events and that many opportunities are
afforded by the more frequent, less serious events to take actions to prevent
the major losses from occurring. Safety leaders have also emphasized that these
actions are most effective when directed at incidents and minor accidents with
a high loss potential.<o:p></o:p></span><br />
<br />
<span lang="EN-US">There is always a large variation between the most serious
and no claim incident, as shown in both pyramids. <o:p></o:p></span><br />
<br />
<span lang="EN-US">In 2003, ConocoPhillips Marine conducted a similar study
demonstrating a large difference in the ratio of serious accidents and near
misses. The study found that for every single fatality there are at least
300,000 at-risk behaviors, defined as activities that are not consistent with
safety programs, training and components on machinery. These behaviors may
include bypassing safety components on machinery or eliminating a safety step
in the production process that slows down the operator. With effective machine
safeguarding and training, at-risk behaviors and near misses can be diminished.
This also reduces the chance of the fatality occurring, since there is a lower
frequency of at-risk behaviors. The variation can be explained by distance or
time – for example, the injury was missed by one second or by one inch. Machine
safety can make a material. The difference in widening the variation, favorably
impacting frequency and severity of claims and, therefore, workers’
compensation premiums. </span><br />

safety and the laws we have to abide by.<span style="mso-spacerun: yes;">
</span>It sounds like common sense to try to prevent accidents, and the last
couple hundred years of evolution since the start of the industrial revolution
has taught us a few things.<span style="mso-spacerun: yes;"> </span>Think about
it… back when people made a living off of the land and the only professionals
were the odd craftsman in the villages, people hardly got hurt, and even then
not that seriously.<span style="mso-spacerun: yes;"> </span>Then as factories
started<span style="mso-spacerun: yes;"> </span>cranking out hammer heads and
coal shovels, workers started getting mangled.<span style="mso-spacerun: yes;">
</span>At first they just wrote it off to stupidity but when chimney sweeps
developed black lungs and coal miners started dropping dead, the government introduced
regulations to force industry to make improvements.<span style="mso-spacerun: yes;"> </span>It was said for years, that these regulations
were written in the blood of many dead workers and it wasn’t until the recent
past that industry became proactive and looked at accident prevention as a
science, with experts trained in the latest developments.<o:p></o:p></span><br />

<span lang="EN-US" style="font-size: 12pt; font-weight: normal;">In the early 1900s <st1:state w:st="on"><st1:place w:st="on">British Columbia</st1:place></st1:state>'s population and workforce
was growing rapidly and had a desperate need for a compensation system.
Forestry, fishing, and mining were the province's core industries — and working
conditions in the woods, on the boats, and in the mines were often shockingly
dangerous. <span style="mso-spacerun: yes;"> </span>In the worst mining disaster
in the province's history, 148 miners lost their lives in a gas explosion at
the Vancouver Coal and Land Co. mine in 1887. Their widows received little
compensation from the company other than basic housing and food.<o:p></o:p></span></div>

Due diligence means that employers
shall take all reasonable precautions, under the particular circumstances, to
prevent injuries or accidents in the workplace. <o:p></o:p></div>

To exercise due diligence, an
employer must implement a plan to <b style="mso-bidi-font-weight: normal;">identify
possible workplace hazards and carry out the appropriate corrective action to
prevent accidents or injuries arising from these hazards</b>. <o:p></o:p></div>
<br />
<div class="MsoNormal" style="margin: 0in 0in 0pt 0.25in;">
<span lang="EN-US" style="font-size: 12pt; font-weight: normal;">The employer must have in place </span><span lang="EN-US" style="font-size: 12pt;"><strong>written OH&S policies, practices, and
procedures</strong></span><span lang="EN-US" style="font-size: 12pt; font-weight: normal;">.
These would demonstrate that the employer carried out workplace safety audits,
identified hazardous practices and hazardous conditions and made necessary
changes to correct these conditions, and provided employees with information to
enable them to work safely. <o:p></o:p></span></div>

that they understand and carry out their work according to the established
polices, practices, and procedures. <o:p></o:p></span></div>

<span lang="EN-US" style="font-size: 12pt; font-weight: normal;">The employer must train the supervisors to ensure
they are competent persons, as defined in legislation. <o:p></o:p></span></div>

<span lang="EN-US" style="font-size: 12pt; font-weight: normal;">The employer must monitor the workplace and ensure
that employees are following the policies, practices and procedures. Written
documentation of progressive disciplining for breaches of safety rules is
considered due diligence. <o:p></o:p></span></div>

<span lang="EN-US" style="font-size: 12pt; font-weight: normal;">There are obviously many requirements for the
employer but </span><span lang="EN-US" style="font-size: 12pt;"><strong>workers also have
responsibilities</strong></span><span lang="EN-US" style="font-size: 12pt; font-weight: normal;">. They have a duty to take reasonable care to ensure the safety of
themselves and their coworkers - this includes following safe work practices
and complying with rules and regulations. <o:p></o:p></span></div>

<span lang="EN-US" style="font-size: 12pt; font-weight: normal;">The employer should have an </span><span lang="EN-US" style="font-size: 12pt;"><strong>accident investigation and reporting system</strong></span><span lang="EN-US" style="font-size: 12pt; font-weight: normal;"> in place. Employees
should be encouraged to report "near misses" and these should be
investigated also. Incorporating information from these investigations into
revised, improved policies, practices and procedures will also establish the
employer is practicing due diligence. <o:p></o:p></span></div>

<span lang="EN-US" style="font-size: 12pt; font-weight: normal;">The employer should document, in writing, all of the
above steps: this will give the employer a history of how the company's
occupational health and safety program has progressed over time. Second, it
will provide up-to-date documentation that can be used as a defence to charges
in case an accident occurs despite an employer's due diligence efforts. <o:p></o:p></span></div>

<span lang="EN-US">Kickback Prevention:</span></h2>
<br />
<span lang="EN-US">Kickback is a sudden reaction to a pinched, bound or
misaligned wheel causing an uncontrolled cut-off tool to lift up and out of the
workpiece toward the operator.<br />
Kickback results from tool misuse and/or incorrect operating procedures or
conditions and can be avoided by taking the following precautions:
Maintain a firm grip with both hands on the unit and position your body and
arms to allow you to resist kickback forces. When wheel is binding, or
when interrupting a cut for any reason, release the trigger and hold the unit
motionless in the material until the wheel comes to a complete stop.
Never attempt to remove the unit from the work or pull the unit backward while
the wheel is in motion or kickback may occur. When restarting a
cut-off tool in the workpiece, check that the wheel in not engaged into the
material. If the wheel is binding, it may walk up or kickback from the
workpiece as the tool is restarted. Support large panels to minimize the
risk of wheel pinching and kickback. Large panels tend to sag under their
own weight. Support must be placed under the panel on both sides, near
